ChronICT (ICT based information and communication resources for patients, their families, and health care providers dealing with chronic disease) will be an interactive information and communication resource for patients, their family, and health care providers dealing with chronic disease. The project focus specifically to the challenges of “living well” with a condition and support activities related to self-care and coping with a chroniccondition. As such we will explore knowledge transfer apply a coping and learning perspective and provide ICT-based resources to augment and supplement existing health care services.
